Listed below is the recommended list of documents for DAPA and DACA.

Proof of Age

  • Birth certificate
  • Passport
  • Consular ID

Proof of Date of Entrance

  • School records/school IDs
  • Vaccination records
  • Medical records
  • Pay stubs
  • Rental/lease Receipts
  • Statement of persons present at the time of entrance (only those with legal status)

Proof of Residency (5 Consecutive Years in the U.S. as of June 15, 2012)

  • Driver’s license/Government ID
  • School ID
  • School Certificates/Awards
  • Tax returns, W-2s, pay stubs (from June 2007 – June 2012)
  • Insurance (car/health/house)
  • School records
  • Medical records
  • Church record
  • Community service records (youth groups, Boy/Girl Scouts, other service organizations, i.e., Boys & Girls Club)
  • Sports records (i.e., Little League)
  • Bank and/or credit card statements (June 2007-June 2012)
  • House title or rent/lease contracts
  • Utility bills (gas, water, electricity, phone, etc.)
  • Memberships (gym, Sam’s Club, etc.)
  • Birth certificates for children born in the U.S.

Proof of Presence in the U.S. on 6/15/2012

  • Sales/ATM receipts on June 15, 2012
  • Pay stubs/time cards on June 15, 2012
  • Letter from employers that confirms presence on June 15, 2012
  • Letter from landlord that confirms presence on June 15, 2012
  • Pictures marked with time/date and taken in an identifiable place in the U.S.
  • Letter from pastor/priest
  • Letter from sports league coach

Proof of Education/Military Service

  • Diploma/Certificate from a U.S. high school
  • GED certification
  • Military discharge papers/military ID
  • Proof of school enrollment/transcript

Proof of No Prior Criminal Activity

  • All records of arrest
  • All court provisions

Various

  • Voluntary Departure order
  • Deportation order
  • Any other immigration documents
