Do the benefits outweigh the risks?

During this first year period, the Institute for Safe Medication Practices (ISMP), a pharmaceutical industry watchdog organization, and publisher of the pharmaceutical industry review Quarterwatch, found 457 “serious adverse events” reported. These serious adverse events ranged from renal failure or impairment, to dehydration, kidney stones, urinary tract infections, to hypersensitivity, which included swelling of the tongue, lips, face or throat, or skin rashes. This review published in the May 2015 edition of Quarterwatch, showed that this drug has a higher number of adverse events reported than 92% of the drugs it regularly watches.

The review concluded that it was uncertain whether this drug does more good than harm over the long term. Other unanswered questions remain, regarding whether long term use of this drug may result in decline in kidney function, increased bone fractures, and increased cardiovascular events.

On May 15, 2015, the FDA released a Safety Communication regarding their finding that SGLT2 inhibitors may cause ketoacidosis. In short the drug may cause high levels of blood acids, known as ketones. This condition is serious enough that the side effects may require hospitalization for several days. The FDA reported that 20 individuals using a SGLT2 inhibitor required emergency room treatment due to diabetic ketoacidosis during the first 15 months Invokana® was on the market.

Ketoacidosis results in symptoms such as difficulty breathing, nausea, vomiting, abdominal pain, confusion, fatigue, and sleepiness. If you are taking Invokana® and are experiencing these symptoms, seek medical help immediately.

Other side effects attributed to Invokana® and SGLT2 inhibitors are:

  • Increased urination
  • Genital myotic infections
  • Urinary tract Infections
  • Increased cholesterol levels

In answering the question, do the benefits outweigh the risks of any particular drug, it is important to establish what positive outcome is desired from the drug being evaluated.
